Your U.S. Bank coverage under the medical, dental, vision, term life and healthcare flexible spending benefits (“group benefits”) ends the first day of the month following your termination date. Within two weeks of coverage ending, you’ll receive a COBRA packet with information on continuing certain benefits, including medical.
If you choose to enroll, Alight (our employee benefits and well-being vendor) will bill you directly each month. COBRA coverage is effective retroactive to the first day of the month after your end date, once Alight receives your payment. Coverage can be extended under COBRA for up to 18 months.

Note: If you or your spouse is age 65 or older, Medicare coordination rules apply, and you can’t receive a benefit under the U.S. Bank medical plan for claims during your COBRA period. Review the Medical and Wellness Summary Plan Description on Your Total Rewards and visit medicare.gov to download the “Medicare and You” handbook. If you or any of your dependents are eligible for Medicare, you cannot receive a benefit from the medical plan if you continue coverage through COBRA.
